Minimum Qualifications

• Bachelor‘s, Master‘s, or Doctoral degree in Physical Therapy from an accredited program approved by the APTA.
• Active Illinois Physical Therapy license required to start the assignment.
• Minimum education requirement: Bachelor‘s degree.
• BLS certification from AHA or ARC may be required.
• Experience or ability to perform in-home physical therapy evaluations and interventions.
• Client-specific requirement: ability to work coverage across Williamson, Jackson, and Franklin counties.

Preferred Qualifications

• Home health experience preferred.
• Experience with OASIS documentation preferred.
• Experience with Homecare Homebase (HCHB) preferred.

Assignment Details

• Shift hours: 2000-0400.
• Weekly hour guarantee: 32 hours.
• On-call/weekend requirement: No.
• Mileage reimbursed between patient visits, from first patient to last patient.
• Coverage areas include Williamson, Jackson, and Franklin counties.
