Registered Nurse - Home Care

Location: Saco, Maine

Assignment Duration: This is a 13-week contract position.

Schedule: Shift: 8:00 AM - 5:00 PM, with a weekend rotation every other weekend, or a permanent schedule available.

Job Description: The Registered Nurse (RN) in Home Care is responsible for providing high-quality nursing care to patients in their home environment. The RN will manage a case load of 20-25 patients, conducting approximately 6 visits per day and utilizing effective communication and collaboration skills to enhance patient care. This role will involve coordinating patient care with a multi-disciplinary team and ensuring compliance with healthcare regulations and standards.

Responsibilities:

• Manage and coordinate care for patients with diverse medical needs, including adults with medical-surgical, oncology, cardiac, neurological, and respiratory issues.
• Perform comprehensive nursing assessments and develop tailored care plans.
• Administer medications, treatments, and other nursing interventions as prescribed.
• Educate patients and families on care plans, medications, and health management.
• Utilize electronic medical records (EMR) systems, specifically EPIC and OASIS.
• Participate in team meetings and contribute to collaborative care strategies.

Required Experience / Certifications / Licensure:

• Minimum of 2 years of experience as a Registered Nurse.
• Active RN license in the state of Maine.
• BLS certification is required.
• Experience with IV therapy, tube feeds, trachs, and other advanced care preferred.
