Job Description:
The RN Case Manager provides care for patients in all the acute care departments, typically remaining in the same area during their contract. They are responsible for overseeing and managing patient care and progress during hospital stays.
Responsibilities:
- Oversee a caseload of 16-20 patients
- Engage in discharge planning and progression of care
- Meet with families to discuss plans of care
Requirements:
- Minimum of 1.5 years general nursing experience
- BLS certification
- Completion of a nursing education program approved by a board of nursing
- Experience in discharge planning and Acute Care Case Management
- Proficiency with EPIC charting system
