Job Title Social Worker Location La Quinta, California, United States Compensation Base Salary: USD $74,612 to $89,535 Full-time Relocation Assistance Available: No Bonus Eligible: Yes Responsibilities Conduct Social Work assessments to determine the psychosocial needs, preferences, and goals of the participants. Actively participate in IDT meetings to develop participant care plans based on assessment findings. Deliver and document social work interventions as outlined in participants' care plans, including arranging necessary resources and services, providing counseling, and offering case management. Collaborate with the primary care physician and other members of the care team to ensure smooth care transitions between settings. Initiate, coordinate, and facilitate care conference meetings to enhance care coordination among care team members, participants, and their support network. Provide discharge planning for participants disenrolling from the program. Job Requirements Masters Degree in Social Work (MSW) required. Minimum of one (1) year of experience with a frail or elderly population. Previous experience coordinating and facilitating care conference meetings. Preferred previous experience assisting individuals with behavioral health and substance abuse issues. #J-18808-Ljbffr
