Occupational Therapist, OT PRN (2319493) As the Occupational Therapist in Home Health, you will be responsible for assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions. Primary Responsibilities Provide services within the scope of practice as defined by state laws governing the practice of Occupational Therapy, in accordance with the plan of care and evidence‑based techniques, in coordination with other healthcare team members. Evaluate the patient’s functional status, all body systems as required for CMS documentation, and occupational therapy needs; consult with the physician for the therapy plan of care. Observe, record, and report to the nurse supervisor and physician the patient’s response to treatment and any changes in condition. Conduct patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests. Required Qualifications Current Occupational Therapy licensure in state of practice. Current CPR certification. Current driver’s license and vehicle insurance, or access to a dependable vehicle or public transportation. Ability to function in any home situation, regardless of client demographics. Preferred Qualifications Demonstrated ability to manage multiple tasks simultaneously. Demonstrated ability to work independently. Good communication, writing, and organizational skills.
