Life on dialysis can be a difficult transition for many patients-and you'll be there to support and advocate for them. In this vital role, you will help patients understand their rights and responsibilities, and guide them in managing the physical, mental, emotional and financial demands of End Stage Renal Disease.

Requirements:

• Meets all state required regulations to practice in the dialysis setting.

• Master's degree in Social Work (MSW) required with a specialization in clinical practice

• Current License in the state of NY

• Minimum of 1 year of experience post licensure

• Demonstrated knowledge of government and private insurance programs

• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days

This position will be open for a minimum of three days.

The Salary Range for the role is $80,000 - $125,000/year. Social Worker Intermediate: $86,000 - $93,000/year.If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position. New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hour
