Now Hiring – RN Case Manager (Utilization Review & Swing Bed) | New Mexico

Pay: $60–$65/hr on W2

Location: Portales, NM

Start Date: ASAP

Contract: 13 Weeks

Shift: Monday–Friday | 8:00 AM – 4:30 PM | 40 Guaranteed Hours

We're seeking an experienced RN Case Manager with expertise in Utilization Review, Discharge Planning, and Swing Bed Coordination for a 13-week contract at a community hospital in New Mexico.

Responsibilities:

✅ Utilization Review using InterQual
✅ Case Management & Discharge Planning
✅ Swing Bed Coordination and Placement
✅ Evaluate admissions for Swing Bed eligibility
✅ Coordinate with physicians and interdisciplinary teams
✅ Prior authorizations for Home Health, SNF, Acute Rehab, and DME
✅ MDS documentation and discharge planning
✅ Educate staff on appropriate level of care and documentation requirements

Requirements:

✔ Active RN License
✔ Minimum 5 years of RN Case Management experience
✔ Strong experience in Utilization Review and Discharge Planning
✔ Experience with InterQual
✔ Swing Bed coordination experience preferred
✔ Experience with Meditech EMR is a plus
