Position: Social Worker, MSW The Social Worker will enhance existing services to include a thorough assessment of families’ long-term goals at intake and development of a service plan to incorporate small achievable tasks leading toward permanent housing, with enrollment and/or referrals to community resources. The role focuses on families in the Creston Avenue Residence, a facility providing housing for chronically homeless single adults, families, and veterans with HUD‑VASH vouchers, and other families meeting income eligibility requirements. Location: Bronx, NY 10468 Minimum Qualifications Master’s degree in Social Work with MSW. Knowledge of child and adolescent development, emotional/behavioral health and mental health, family dynamics, parent-child relationships, diagnostic classifications. Experience and skills in strengths‑based, solution-focused, and family-centered practice; working with diverse cultures and ethnicities; knowledge of child welfare policies, regulations, safety, permanency, and well-being. Principal Responsibilities Manage a caseload. Complete assessment summary and determine the type of intervention needed for each family. Develop individualized service plans focused on recovery and wellness. Maintain service documentation, including client progress notes, observations, interactions, and incident reports when necessary. Provide on‑call crisis intervention support. Participate in service coordination activities, including vocational and educational services and supporting recreational/social activities. Maintain the program’s resource directory. Conduct outreach to community-based organizations. Refer families to appropriate community providers to address family needs.
