For more information, visit Job Title Advanced Practice Provider - Family Health Center Location Acclaim Family Health Center Job Description Acclaim Multispecialty Group, a partner with JPS Health Network, is seeking a motivated and ambitious advanced practice provider to join our General Family Medicine team. This is an opportunity to treat complex medical issues for patients who need it most, within a county hospital system and urban teaching hospital that emphasizes education and scholarly activities. The hospital is located in the heart of The Magnolia Village and Medical District of Fort Worth, a vibrant city with cultural amenities and a thriving downtown. Job Summary Outpatient Clinic Monday–Wednesday 7:00 AM–7:00 PM, rotating Saturday coverage Patient panel – Primary Care – Adult and Pediatric Duties and Essential Job Functions Coordinate multidisciplinary care for the patient panel to optimize care. Review patient medical records to diagnose, treat, and provide care management and education to patients and families. Make appropriate patient referrals. Monitor national health guidelines and those established by JPS Health Network and Acclaim Multispecialty Group for patient care. Provide evidence-based and compassionate care to improve patient outcomes and satisfaction; demonstrate patient-centered and patient-valued behavior. Maintain a collaborative environment with referring physicians, residents, medical students, advanced practice professionals, staff, and support staff to provide quality patient care. Support academic, educational and research endeavors. Attend performance improvement programs and peer review committee meetings; actively participate in the process. Build a positive brand and patient/customer experience for those who encounter JPS Health Network and Acclaim Multispecialty Group; engage with staff, vendors, partners, patients, stakeholders, and community leaders. Serve as a resource to the community. Performs other job-related duties as assigned. Knowledge, Skills and Abilities Comfortable working in a large public/county facility. Working knowledge in the assigned area, with competency in assessment techniques, diagnosis and treatment processes relevant to the specialty. Strong interpersonal, verbal and written communication skills to work effectively with health care professionals and the community. Self-directed with initiative to develop and participate in community activities and medical outreach programs related to the specialty. Qualifications Required Education and Experience: Graduate of an accredited Nurse Practitioner program with an active Texas license; current Texas APRN license with ACNP certification. or Graduate of an accredited Physician Assistant program with an active Texas license. Required Licensure/Certification/Specialized Training: Current, unrestricted medical license to practice in the State of Texas. Unrestricted work authorization to work in the United States.
