As a Licensed Practical Nurse (LPN), you will play a vital role in our team's mission to improve the lives of our residents. We are seeking a compassionate and skilled LPN to join our team of world-class healthcare professionals.

As an LPN at Heritage Lakeside, you will be responsible for monitoring, planning, and implementing the nursing care of our residents under the direction of a Registered Nurse (RN). You will ensure continuity of care by providing direct care as well as supervising the care given by Certified Nursing Assistants (CNAs) and support staff members. Additionally, you will participate in resident and family education, and serve as a resident advocate with a focus on our facility's mission.

Responsibilities:

• Monitor, plan, and implement the nursing care of residents within the facility under the direction of a RN
• Ensure continuity of care of residents between shifts by providing direct care and supervising the care given by CNAs and support staff members
• Participate in resident and family education
• Serve as a resident advocate with focus on the facility's mission

Requirements:

• Current/valid Wisconsin LPN license/registration
• 0 years of experience (entry-level position)
