Description

• Conduct evaluations and create tailored treatment plans for patients in both inpatient and outpatient settings

• Guide and educate patients and their families on therapeutic objectives, discharge planning, and home exercise programs
• Maintain thorough documentation of evaluations, treatments, and patient progress in line with facility and legal requirements
• Participate in care team rounds and interdisciplinary meetings to ensure synchronized patient care

Education

• Doctorate in Physical Therapy (DPT) or a comparable degree from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification preferred
