Epic Healthy Planet Implementation Analyst

Job Description In this role, the analyst serves as a key leader in the implementation and optimization of Epic Healthy Planet for a mid‑sized hospital or health system. They partner closely with population health leaders, care management teams, clinical stakeholders, quality departments, and IT leadership to design workflows that support value‑based care initiatives, preventative care programs, and improved patient outcomes. Lead Healthy Planet workstreams throughout the implementation lifecycle, including planning, design, build, testing, go‑live, and post‑production support. Facilitate requirements‑gathering sessions with operational and clinical leaders to translate business needs into Healthy Planet solutions. Configure and optimize registries, Compass Rose programs, care gap workflows, risk stratification tools, and quality measures. Support HEDIS and population health initiatives by building reporting tools, identifying care gaps, and monitoring quality metrics. Collaborate with analytics and quality teams to develop dashboards, Reporting Workbench reports, and population health insights. Coordinate system testing, workflow validation, end‑user training, and go‑live readiness activities. Act as the primary Healthy Planet subject matter expert for assigned projects, providing guidance to stakeholders and mentoring junior analysts when needed.
