The Family Support Partner (FSP) is a parent/caregiver of a child with emotional or behavioral challenges who has successfully navigated the system. This lived experience allows the FSP to have a unique ability to engage with families receiving services and support them with empathy and understanding. The FSP will work within the Clinical Management Department. 

The FSP will primarily provide support and guidance to caregivers of youth referred, actively receiving treatment and/or discharged from any child acute and/or residential treatment program. Additional responsibilities may be assigned to support youth and families receiving children’s community-based services. The FSP will make outreach calls, participate in inter-agency meetings, and conduct site visits along with team members. The FSP will assist in further fostering family driven care within the organization and across provider network.   

Essential Functions : 

• Conducts outreach and builds rapport with family members of youth referred to child acute and residential treatment programs. Remains connected to the family pre, during and post treatment.
• Assists families in completion of family satisfaction surveys.
• Collaboration with cross system involvement for youth and family (DHS, Community Umbrella Agencies, Probation, Child Advocate Attorneys/Social Workers, School District). 
• Participation in inter-agency meetings.
• Attends provider meetings as needed.
• Attends weekly rounds discussions and team meetings.
• Maintains excel grid of outreach and interventions.
• Completes documentation in youth’s electronic health record. 
• Works with Clinical Care Managers and other members of the team to facilitate care coordination and connection to meet youth and family needs/goals.
• Communicates and responds to internal and external phone calls/emails in a timely manner. 
• Conducts site visits to provider agencies, as needed.
• Preparation and participation in weekly supervision.
• Identifies and connects families to resources addressing social determinants of health (SDOH).
• Monitors/identifies potential barriers to treatment and works with the treatment team to resolve them.
• Collaboration with other CBH Clinical teams to ensure continuity of care. 

Position Requirements: 

• Education:  High School Diploma/GED preferred
• License/Certification:  

• PA Child Abuse Clearance required
• Family Peer Support Specialist Certification (can be achieved within 6 months of hire)

• Relevant Work Experience: 1 - 2 years of experience assisting families in navigating child-serving systems. 
• Ability to travel throughout Philadelphia and surrounding counties.

Skills:

• Familiar with services for children in the managed care system
• Excellent interpersonal and collaboration skills
• Excellent verbal and written communication skills
• Proficiency with Microsoft Office, especially with Excel, and ability to type 20 words per minute
• Strong organizational skills 
• Ability to multi-task in fast-paced environment
• Ability to work independently and as part of a team
• Compliant with HIPAA regulations

Work Conditions (including travel, overtime required, physical requirements and occupational exposure): 

• Must be able to travel within Philadelphia and surrounding counties.
• Regularly requires walking, standing, stooping, bending, sitting, reaching, pushing, pulling and/or repetitive wrist/hand movements for various lengths of time throughout the day.
• Ability to utilize computers and office equipment to complete daily work responsibilities.
• Regularly works inside in areas that are adequately lighted and ventilated.  Some fluctuation in temperature. No protective equipment routinely needed.
• Must be able to adapt to continuous changes/demands of the job.

Philadelphia Residency Requirement:

• The successful candidate must be a current Philadelphia resident or become a resident within six months of hire.
