Registered Nurse (RN) - Home Health

This is a 13-week contract position.

AM shifts, 5 days per week, working 8-hour shifts.

We are seeking a dedicated Registered Nurse (RN) with recent Home Health experience to provide quality patient care within specified coverage areas. The ideal candidate will be responsible for conducting various nursing tasks, managing patient documentation, and coordinating care according to best practices.

Responsibilities:

• Assess patient needs and develop individualized care plans.
• Conduct Start of Care and Resumption of Care assessments.
• Perform routine visits, recertifications, and discharges.
• Document patient progress accurately and in a timely manner.
• Maintain compliance with healthcare regulations and organizational policies.

Required Experience / Certifications / Licensure:

• Active RN license.
• Minimum of 30 points required for care services provided.
• Experience with HomeCare HomeBase (HCHB) is required. Candidates may be considered without HCHB experience at a reduced rate for the first 2 weeks if training is arranged.
• Recent Home Health experience is mandatory.
