Home Health Registered Nurse The Home Health Registered Nurse administers skilled, quality nursing care to patients on an intermittent basis in their place of residence. This position coordinates and directs the home health patient's care based on individual patient needs and requires advanced pediatric assessment, teaching, and decision‑making skills. The work is performed in accordance with physician orders and the plan of care under the direction of the Administrator or a designee. The Home Health RN provides comprehensive assessment, planning, implementation, and evaluation for a caseload of home health patients in accordance with the plan of care. The RN makes the initial evaluation visit and regularly re‑evaluates the patient’s nursing needs by assessing physical, functional, psychosocial, and cognitive status using interview observations and physical exam techniques. The RN completes and submits OASIS assessments, reassessments, transfers, resumptions of care, discharges, and significant changes in condition in accordance with agency‑defined time frames for applicable patients. Based on evaluations, the RN determines the amount and type of nursing needed by each individual patient and establishes individualized, measurable goals in consultation with the patient, family, and other health‑care providers. Minimum Education and/or Training: Graduate from an accredited school of nursing. RN required. BSN preferred. Minimum Experience: Four (4) years of nursing experience required. Pediatric experience preferred. Fluent in Spanish preferred. Licensure, Registration and/or Certification Required by Law: (LC: RN‑TN) Must possess a current Tennessee State Board of Nursing license if primary residence is Tennessee or a nurse‑licensure non‑Compact state. If operating a motor vehicle for St. Jude business, must have a valid driver’s license from the employee’s state of residence and a valid personal insurance policy on the vehicle that meets the requirements for the state of Tennessee.
