Case Manager

Humana is seeking a Case Manager to support members across Green Lake, Marquette, Adams, and Waushara counties. In this role, you will conduct comprehensive assessments, develop individualized care plans, and coordinate medical, behavioral, and social services to improve members’ health outcomes. You will collaborate closely with physicians, nurses, social workers, and community partners to connect members with the right care and resources, while educating them on benefits and self-management strategies.
