Our care model focuses on coordinating services, closing care gaps, and preventing avoidable emergency department visits and hospitalizations.We are a family practice serving patients across the full lifespan, from newborns through end-of-life care.Our Medical Assistants (MAs) and Ambulatory Care Team (ACT) operate within a pod-based structure. This model allows efficient and collaborative coverage of key responsibilities, including:Chart preparationPatient roomingVital sign collectionPoint-of-care (POC) testingIn-basket managementThis coordinated approach supports high-quality, patient-centered care while improving workflow efficiency and team accountability.Weekly Hours of work: Monday 6:45 am to 3 pm; Tuesday 9 am to 5 pm; Wednesday 6:45 am to 12 pm; Thursday 8:45 am to 7 pm; Friday 9 am to 4 pm.Corewell Health operates over 300 outpatient clinics across Michigan, bringing a multitude of care options to the communities that we serve.
