Additional Job Description: POSITION SUMMARY : The Advanced Practice Professional (APP) has coordination and clinical skills required for the comprehensivemanagement of Cardiology patients, ( including biological, socialand psychological assessments). The APP coordinates involveme nt ofappropriate medical teams necessary to develop a comp rehensiveplan for the patient, reviews the plan of care with the othermembers of the clinical team, participates in directing that care,performs diagnostic/therapeutic procedures, peri - proceduralservices, and coordinates discharge planning (as applicable) . TheAPP participates in the education of residents and medical/nursingstudents. Provide healthcare services consistent with state law and therequirements of the professional licensing or certificationauthority and/or hospital privileges, where applicable . MINIMUM QUALIFICATIONS FOR NURSE PRACTITIONER : EDUCATION, CERTIFICATION, AND/OR LICENSURE : Graduate of a CCNE accredited nursing program with Master’sDegree in Nursing National Certification by one of the recognized APRNCertification Centers Licensure or eligibility for licensure, as a professionalRegistered Nurse and Advanced Practice Registered Nurse in WestVirginia. EXPERIENCE : Acute care, cardiology or electrophysiology experience preferred. Eligible new graduates will be considered. MINIMUM QUALIFICATIONS FOR PHYSICIAN ASSISTANT : EDUCATION, CERTIFICATION, AND/OR LICENSURE : Bachelor’s Degree as a Medical/Surgical (as applicable)Physician Assistant required. Graduate of an ARC-PA approved Physician Assistant (PA) trainingprogram required. Certification by the National Commission for Certification ofPhysician Assistants required. Must be licensed or eligible for licensure by the WV Board ofMedicine. EXPERIENCE : Acute care, cardiology or electrophysiology experience preferred. Eligible new graduates will be considered. PREFERRED QUALIFICATIONS : EDUCATION, CERTIFICATION, AND/OR LICENSURE : Masters level education as Medical/Surgical (as applicable)Physician Assistant preferred. CORE DUTIES AND RESPONSIBILITIES : The statements describedhere are intended to describe the general nature of work beingperformed by people assigned to this position . Manages comprehensive and individualized care for Cardiologypatients and their families in the inpatient phase ofhospitalization and immediate post discharge in the ambulatorysetting. Completes daily rounds ; consults and/or pre-procedureevaluations. Completes integrated assessments. With the collaboration of Card iology faculty or by followingestablished clinical Guidelines and Protocols, orders additionaltests or consults as deemed appropriate . Performs, interprets and /or communicates results of diagnostictests following established clinical g uidelines and scope ofpractice in the inpatient and outpatient setting Consults and communicates with clinic and hospital staffregarding abnormal findings in the evaluations of patients. Performs succinct comprehensive clinical evaluations anddocuments findings in medical record . Performs necessary procedures following established clinicalguidelines and Protocols and scope of practice (SOP). Facilitates patient discharge by coordinating appropriateservices and tests prior to discharge necessary for outpatientfollow-up appointments in the clinic (as applicable) . Develops a comprehensive individualized plan of care for electrophysiology patients, which includes teaching, discharge planningand necessary referral to other members of the health care team asnecessary. Demonstrates expertise in caring for patients across the agecontinuum of their educational experience and certification. Identifies unstable or complex Cardiology patient caresituations and facilitates appropriate intervention in the clinicand/or hospital setting. Recognizes emergency situations and initiates effectiveemergency care. Evaluates Card iology patients and serves as a coordinator forfollow up procedures and tests in the ambulatory setting (asapplicable). Assists in the collection of outcome measures (asapplicable). Demonstrates critical thinking and diagnostic reasoning skillsin clinical decision making by the identification, evaluation andmanagement of the individual needs of patients/families. Demonstrates the ability to utilize equipment in a safemanner. Bases decisions made/actions taken on the relevant scientificprinciples, established standards of care and/or researchfindings. Utilizes teaching/learning theory and appropriate teachingstrategies in the provision of education to nursing staff, hospitalstaff, students, patients, families and the community. Promotes an environment that facilitates learning by respondingto appropriate patient/family preferences, priorities and readinessto learn. Updates own knowledge base throughout the year. Completes all identified clinical competencies. Serves as a resource person for staff caring for the Cardiology patient. Provides educational programs to professional andparaprofessional personnel in area of expertise. Participates in community programs, distribution of literature,as appropriate in area of expertise. Documents the plan of care, interventions and evaluations in aclear and concise manner for every patient visit in the patient’smedical record following approved hospital/UHA policies andcompliance standards for documentation . Collaborates with other leaders and staff to effectivelyachieve patient outcome based improvements in care. Provides leadership in the development and implementation ofchanges in the cardiology / electrophysiology practice thatpositively impact patient outcomes . Works collaboratively with medical and nursing staff tofacilitate creative problem solving and provide professionalsupport. Participates and initiates inter-disciplinary approaches topatient care, program development and education . Serves as aprofessional role model. Promotes and practices innovation of the expanded role in thedelivery of care to Card iology patients and families throughoutthe inpatient hospitalization, and the ambulatory setting. Participates and provides leadership for committees and taskforces. Documents hours and submits billing reports, as appropriate. Re views and interprets Card iology data from sources such as patient registries, clinical and financial reports, length of stay,access to care, and cost per case reports and recommends/implementschanges as necessary. Participates in clinical trials and research studies with thePrincipal Investigator. Assi sts Cardiology Faculty, Administrators and Directors inthe collection of outcome measures. Other duties/projects as assigned. PHYSICAL REQUIREMENTS : Heavy/ Hard Work: Work requires strength and/or stamina,lifting, moving, stooping, reaching, positioning of patients,standing, walking, and moving of patients, materials and equipmentweighting 40+ pounds.
