Jobtailor is seeking a Registered Nurse for discharge planning and care coordination in Falls Church, VA. You will work with patients and families, coordinate post-acute transitions, and analyze utilization data to improve outcomes. Requires BLS, Virginia RN licensure eligibility, and 1 year of case management or clinical care experience; BSN preferred or ADN with plan for BSN completion.
