Job Summary:

Seeking a skilled Physical Therapist with extensive experience in Oasis, home health, and hospice care. The ideal candidate will be adept at managing patient visits in a travel-intensive outpatient setting, demonstrating flexibility to adapt to scheduling changes.

Responsibilities:

• 
  Evaluate patients' physical health and develop personalized treatment plans.
  
  
• 
  Conduct home visits, traveling between patient locations to provide care.
  
  
• 
  Adjust schedules as needed to accommodate patient needs and changes.
  
  
• 
  Document clinical findings and treatment progress accurately in the EMR system.
  
  
• 
  Collaborate with care teams to coordinate patient care and ensure quality standards.
  
  
• 
  Maintain compliance with all healthcare regulations and facility policies.
  
  

Qualifications:

• 
  Proven experience with Oasis documentation, home health, and hospice environments.
  
  
• 
  Ability to travel regularly between patient locations for home visits.
  
  
• 
  Proficiency with the HealthWyse EMR system.
  
  
• 
  Strong organizational skills to manage and adapt to changing schedules.
  
  
• 
  Valid state licensure as a Physical Therapist.
  
  
• 
  Excellent communication and interpersonal skills.
