Position Overview:

Develop individualized treatment plans to help residents regain independence, prevent decline, and safely participate in daily activities while enhancing quality of life.

Key Responsibilities:

• Assessing and interpreting patient evaluations and test results.

• Developing treatment plans using a variety of treatment techniques.

• Creating personalized rehabilitation programs for patients.

• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.

Preferred Qualifications:

• Active/Valid Illinois license in Physical Therapy

• SNF/Long term care experience preferred.

• Current knowledge of treatment practices

• Knowledge of PCC Electronic Documentation System preferred.
