Nursys (both as Certification and License) The Prime Resume, Prime Reference #1, and Prime Reference #2 are required for submission, while the rest are required for onboarding. Optional items include the Name Verification Affidavit, Fit Test, Positive PPD- Negative Chest X-Ray, and Prime COA.
