At Lincoln Meadows Care Center, you'll join a collaborative interdisciplinary team dedicated to helping our residents achieve their highest level of independence, mobility, and quality of life.If you're passionate about patient-centered care, enjoy working in a supportive skilled nursing environment, and want to be part of a team that truly values your expertise—we'd love to meet you!Why Join Lincoln Meadows Care Center?Competitive pay: $55.00–$65.00/hour DOEFull-time, stable scheduleSupportive interdisciplinary teamModern therapy programs and equipmentOpportunities for professional growth and leadershipMeaningful work with a rewarding resident populationPositive, team-oriented cultureWhat You'll DoAs a Staff Physical Therapist, you will:Evaluate residents and develop individualized treatment plans based on physician ordersImprove residents' strength, mobility, balance, endurance, and functional independenceSupervise Physical Therapy Assistants (PTAs), rehab aides, and studentsTrain and support Restorative Nursing AssistantsCollaborate with physicians, nursing staff, residents, families, and the interdisciplinary teamParticipate in care conferences, utilization review, and rehabilitation meetingsMaintain accurate, timely, and compliant clinical documentationEducate residents and families on home exercise programs, fall prevention, and safety techniquesRecommend appropriate mobility equipment and adaptive devicesProvide facility-wide education and in-service trainingSupport quality improvement initiatives and rehabilitation program developmentMaintain a safe treatment environment while following infection control and safety standardsWhat We're Looking ForCurrent California Physical Therapist license in good standingBachelor's, Master's, or Doctorate in Physical TherapyCPR Certification (if required by licensure)Skilled Nursing Facility experience preferred, but not requiredExperience with electronic medical records such as Net Health and PointClickCare is a plusStrong communication, documentation, and teamwork skillsA compassionate clinician committed to outstanding resident outcomesPhysical RequirementsThis position requires standing, walking, lifting, pushing, pulling, transferring residents, and safely operating therapy equipment throughout the workday.
