Responsibilities
- Evaluate residents’ functional abilities, including Activities of Daily Living, mobility, cognition, and psychosocial factors
- Develop and implement individualized occupational therapy treatment plans based on evaluations and physician orders
- Provide direct OT services in accordance with state and federal regulations and facility policies
- Monitor and document resident progress; adjust treatment plans as needed to meet goals
- Educate residents, families, and caregivers on strategies to promote safety and independence
- Supervise and provide direction to Certified Occupational Therapy Assistants (COTAs) and therapy aides as appropriate
- Ensure accurate, timely documentation in the electronic medical record
- Collaborate with the interdisciplinary team; participate in care planning meetings
- Maintain licensure and continuing education requirements as mandated by state regulations
- Support quality improvement initiatives and comply with CMS and facility guidelines

Requirements
- Graduate of an accredited Occupational Therapy program
- Current Occupational Therapist license in the state of practice
- Prior SNF or geriatric rehabilitation experience preferred
- Strong clinical reasoning, communication, and organizational skills
- Ability to work effectively in a team-oriented environment
