Clinical Patient Navigator (CMA certified)

Job Overview The Clinical Patient Navigator manages front‑office functions and serves as a dual administrative/clinical role to support the efficient operation of an outpatient wound care center. The position supports provider workflow, patient communication, and overall patient care quality. Key Responsibilities Coordinate with Center leadership to maximize daily patient census. Actively participate in staff meetings and support key functions within the Center. Greet patients and visitors, answer and route calls, and maintain all patient communication needs—including scheduling, rescheduling, and appointment reminders. Work and schedule for providers according to the care continuum model. Obtain and verify patient insurance information, including pre‑certifications and pre‑authorizations, and enter data into appropriate databases. Collect and enter patient charges, verify and reconcile charges, prepare monthly patient survey data, and submit documentation to providers. Assemble new patient charts, maintain and file existing charts, and spot‑check charts for data completeness and signatures. Coordinate and schedule ancillary testing with other hospital departments and arrange patient transportation as needed. Maintain office equipment and supplies as needed, and manage medical supplies as directed. Under the direction of the RN/Provider, provide wound‑related patient care as approved by hospital protocols and policies. Assist with intake and discharge within scope of practice and according to hospital bylaws, after completing required skill competencies. Perform hyperbaric oxygen therapy related duties as required, after completing Healogics hyperbaric training and competency. Assist in Vein Clinic within scope of practice after completing required training. Gather patient information and necessary equipment and supplies for the RN or provider. Report quality of care issues to the Clinical Nurse Manager. Communicate with team members regarding therapeutic interventions to ensure quality of care. Function as a Documentation Assistant (scribe) in accordance with Healogics policy. Perform other duties as required. Required Education, Experience and Credentials High School Diploma or General Education Development (GED); Associate’s degree preferred. Certified Medical Assistant (CMA) credential from a program approved by CAAHEP or ABHES. Current CMA Certification licensed in the state of employment, maintained throughout employment. Minimum of two (2) years of office administration experience, preferably in a medical setting (acute hospital, rehabilitation hospital, or skilled nursing unit). Prior medical coding experience preferred. Required Knowledge, Skills, and Abilities Demonstrated competency in Basic Life Support (BLS). Proficient in Microsoft Office (Word, Excel, Outlook). Strong customer, interpersonal, and communication skills—both oral and written. Organizational and time‑management skills. Basic math skills and attention to detail. Ability to maintain confidentiality. Ability to work in a fast‑paced environment and manage multiple projects simultaneously. Team‑work and collaboration skills. Physical Demands Stationary position for extended periods of time (4+ hours). Viewing computer screen for extended periods of time (4+ hours). Keying frequently on a computer for 4+ hours. Reading. Lifting/moving items up to 75 pounds with equipment assistance. Pushing/pulling, bending/stooping. Reaching, grasping, and touching with hands. Detecting sounds by ear. Repetitive motions. Work Environment Primarily indoors patient care environment. Exposure to hazards (blood‑borne pathogens, toxic chemicals, flammable gases, etc.). Exposure to mechanical equipment.
