We are seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join our senior living community. This individual will be responsible for providing high-quality nursing care, administering medications, coordinating resident care, maintaining accurate documentation, and ensuring residents' physical, emotional, and social well-being. The ideal candidate is passionate about serving the elderly population and thrives in a collaborative team environment.

Day-to-Day Responsibilities

• Administer medications according to physician orders and maintain accurate medication records.
• Complete resident assessments and assist with the development of individualized service plans.
• Monitor changes in residents’ health conditions and ensure concerns are evaluated, documented, and addressed.
• Perform nursing procedures within the appropriate scope of practice and in accordance with state regulations.
• Respond to medical emergencies and provide first aid when needed.
• Review resident records to ensure documentation accurately reflects concerns, interventions, and outcomes.
• Communicate with residents, families, physicians, nursing staff, and other healthcare providers.
• Support infection-control practices and maintain a safe, healthy environment.
• Assist with supervising wellness department employees who provide daily resident care.
• Provide orientation and ongoing training to team members as requested.
• Prepare written and verbal reports within required timeframes.

REQUIRED SKILLS AND EXPERIENCE

• Active LPN license in good standing with the applicable state licensing authority.
• Ability to administer medications and perform nursing procedures within the appropriate scope of practice.
• Strong clinical documentation and organizational skills.
• Ability to recognize and respond to changes in a resident’s condition.
• Ability to work scheduled hours and remain available by phone for questions involving medical concerns.

NICE TO HAVE SKILLS AND EXPERIENCE

• 1-3+ years of nursing experience in assisted living, senior care, skilled nursing, or long-term care.
• Experience developing resident care plans.
• Supervisory or training experience.
• Knowledge of state regulations related to senior living communities.
• Experience participating in resident and family care conferences.
• Experience with electronic medical records (EMR/EHR) systems
