PER DIEM RN-REGISTERED NURSE

Overview VNAcare, a not-for-profit leader in home care, is looking for dedicated healthcare professionals to join our hospice team in Apple Valley and surrounding areas. Position Hospice RN (Per-Diem) Responsibilities Perform the initial assessment and periodic re-evaluation of nursing needs of the patient/family, including special pain and spiritual assessments. Observe, evaluate, and report significant changes in physical and psychosocial condition to the physician and the Hospice Interdisciplinary Team; discuss client problems with supervisor; assist with updates to the patient’s plan of care within two days of admission or condition change. Attend and document conferences with therapists and volunteers as needed for coordinated care. Document care while reflecting agency policy and procedures; complete client care plans; provide clinical supervision; cost‑effectively manage patient’s Hospice benefit within Hospice guidelines. Qualifications / Requirements Current RN license in the State of CA. Minimum one year of acute care experience.
