As a Home Care RN, you'll provide comprehensive, patient-centered nursing care to individuals recovering from illness, injury, surgery, or managing chronic medical conditions. You'll perform skilled assessments, develop individualized plans of care, provide hands-on clinical care, educate patients and caregivers, coordinate services with physicians and the interdisciplinary team, and help patients achieve their personal health goals in the comfort of their own homes.

This role offers the unique opportunity to build meaningful relationships with patients and families while practicing with a high level of autonomy. You'll manage a diverse caseload, perform skilled nursing visits, monitor patient progress, provide medication and disease management education, coordinate community resources, and serve as a trusted advocate throughout each patient's healthcare journey. In accordance with national standards of nursing practice and within the guidelines and policies and procedures of both MHCAH and the Maine State Board of Nursing, the Home Care RN provides highly reliable, evidenced based, patient-family centered quality care through assessment, diagnosis, outcome identification, planning, implementation, documentation, and evaluation of the effects of nursing care. Care delivery and coordination of patient and family care begins with the initial contact with the health care system and may continue across the continuum. Services may be provided on an outpatient, and/or community basis.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

• Education: Graduation from an accredited school of nursing. A Bachelor of Science Degree in Nursing is preferred.
• License/Certifications: Current license to practice as a Registered Professional Nurse in the State of Maine. Current certification in Basic Life Support for Healthcare providers. Current, valid drivers license to operate a motor vehicle, with vehicle available for work covered by liability insurance as specified by agency policy.
• Basic knowledge of the research base, theory, practices, principles, and processes of professional nursing.
• Interpersonal and communication skills to interact effectively with patients, families, and health care team members.
• Analytical ability sufficient to identify changes in patients' conditions and to initiate appropriate action.

The base range is an estimate and does not reflect the full value of our total compensation package.

The pay range for this position is $56,858.88 to $102,935.04 per year.

Compensation for part-time positions is determined on a prorated basis consistent with the scheduled hours and applicable full-time equivalent (FTE).
