• Conduct comprehensive clinical assessments to understand the individual needs of patients, documenting their mental health status and developing tailored treatment plans.

• Administer evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) or mindfulness strategies, to address the psychological needs of patients dealing with grief and terminal conditions.

• Maintain accurate and timely clinical records within our electronic health record (EHR) system, ensuring compliance with regulations and quality standards.

• Collaborate with a multidisciplinary treatment team including physicians, nurses, and chaplains, to provide a cohesive treatment approach that meets the holistic needs of our patients.

• Provide education and emotional support to patients and families, offering resources to help them manage the complex emotional and psychological aspects of end-of-life care.

• Stay current with the latest research and developments in mental health and hospice care, engaging in ongoing professional development to enhance your skills.

Your primary focus will be on patients receiving hospice care, who require specialized support in managing anxiety, depression, and other mental health conditions as they confront serious illness. You will also work closely with family members, providing them guidance and support during a challenging time in their lives. Understanding the unique dynamics of end-of-life care and the emotional landscape of both patients and their families is crucial in this role.

• A Master's degree in Social Work (MSW) from an accredited institution.

• A valid LCSW license in the state of Oregon is required prior to your start date.

While a minimum of two years of relevant experience is required, candidates with a background in hospice care, palliative care, or experience working with terminally ill patients are especially well-suited for this role. Experience in crisis intervention and grief counseling will also be highly beneficial.

The salary for this position ranges from USD 80,000 to 100,000 annually, contingent upon experience and qualifications.
