Develop, implement, and update individualized plans of care. Administer medications, wound care, IV therapy, and other skilled nursing treatments as ordered. Monitor patient progress, identify changes in condition, and communicate updates with physicians and the interdisciplinary care team. Educate patients and caregivers on disease management, medications, treatments, and safety in the home. Complete accurate, timely clinical documentation in accordance with agency, state, and federal guidelines. Coordinate care and facilitate appropriate referrals to ensure optimal patient outcomes.
