Minimum Requirements

Bachelor's, Master's or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
Active California Physical Therapist license required to start the assignment.
BLS Certification may be required from AHA or ARC.
Experience delivering home health physical therapy services preferred.
Ability and willingness to travel within assigned county territory (Area #12 San Jose and Area #11: Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos). Additional travel outside the area may be required.
Willingness to provide weekend coverage when required preferred.
Productivity expectations: ability to meet assigned weekly visit productivity (client describes target of at least 27 points per week as a performance expectation; this is client-specific and reflects workload expectations).
Previous experience managing a caseload in a fast-moving outpatient or home health environment preferred.

Assigned Territory: Area #12 (San Jose) and Area #11 (Santa Clara, Sunnyvale, Cupertino, Milpitas, Mountain View, Campbell, Los Altos, Saratoga, Los Gatos).
Travel Requirement: May require travel outside assigned territory and occasional weekend coverage.
Assignment Length: 13 week assignment.
