Key Responsibilities of the RN - Case Management - Travel

• Perform comprehensive assessments of high-risk members.
• Collaborate with primary care providers to implement individualized care plans.
• Continuously reassess cost-efficient, appropriate levels of care.
• Coordinate care transitions and engage specialty resources as needed.
• Maintain detailed records of clinical, functional, and fiscal outcomes.
• Act as a patient advocate, protecting privacy and confidentiality.

Requirements of the RN - Case Management - Travel

• RN License for WI
• Certified CPR/BLS
• Wound Care Experience
• Valid Driver's License and Reliable Vehicle
• Valid Car Insurance
• Case Management certification
• Experience in the use of Electronic Medical Records such as HCHB or Matrix Care
