Registered Nurse - Case Management - Hospital

Job Description

Job Summary:

Seeking an experienced Travel RN Case Manager to coordinate patient care and facilitate transitions across acute, home health, and skilled nursing settings. This role requires advanced assessment and documentation skills within a fast-paced hospital environment.

Responsibilities:

• Assess patient needs and develop individualized care plans using Meditech and Midas systems.
• Collaborate with multidisciplinary teams, including physicians, nurses, and social workers.
• Coordinate patient discharges and transfers to appropriate care facilities.
• Document case management activities accurately in Meditech and Midas.
• Educate patients and families regarding care plans and post-discharge instructions.
• Maintain compliance with hospital policies and certification requirements.

Qualifications:

• Valid RN license with at least two years of clinical experience.
• Demonstrated proficiency in Meditech and Midas documentation systems.
• Strong assessment skills, including DCP and InterQual expertise.
• Experience in hospital case management, discharge planning, and transitions of care.
• Ability to work 8am-4:30pm with weekend rotation, no floating policy.
• Certification in case management is a plus.
