The environment fosters professional growth and empowers therapists to cultivate strong relationships with patients and their families.Key ResponsibilitiesConduct comprehensive evaluations and assessments to create tailored treatment programs based on individual patient needs as referred by physicians.Implement evidence-based therapeutic interventions in home care settings, ensuring the comfort and safety of patients.Collaborate with a diverse team of healthcare professionals to maximize patient outcomes, sharing knowledge and supporting each other in continuous improvement efforts.Educate patients and their caregivers about treatment plans, exercises, and adaptive techniques to foster independence and functionality.Utilize advanced physical therapy techniques and modalities to facilitate recovery and improve patients' mobility and strength.Maintain meticulous documentation of patient progress and treatment adjustments in compliance with healthcare regulations and organizational policies.Meet productivity goals and contribute to the overall success of the practice by providing exceptional customer service and responsiveness to patient needs.Patient Population / Client FocusThis position involves working with a diverse clientele that includes orthopedic patients recovering from surgeries, pediatric patients with developmental challenges, neurologic patients dealing with conditions like stroke or MS, and athletes recovering from sports-related injuries. The ability to assess and adapt to various patient conditions and age groups is crucial in delivering effective home health physical therapy.QualificationsA Doctorate in Physical Therapy (DPT) from an accredited institution is required.A valid state license as a Doctor of Physical Therapy (DPT) from the Physical Therapy Examining Committee.A minimum of two years of experience in a clinical setting, preferably focusing on home health care.Current CPR certification is necessary to ensure patient safety during all treatments.Excellent written and verbal communication skills to effectively interact with patients and team members.Proficiency in computer skills, including word processing and electronic medical records systems.A strong desire to build lasting relationships with patients and contribute positively to their healthcare journeys.Preferred ExperienceWhile a minimum of two years of experience is required, candidates with experience in home health physical therapy are particularly encouraged to apply. Knowledge of pediatric, orthopedic, and neurologic rehabilitation will be considered an asset as these are common patient demographics in our practice.Compensation and BenefitsThe salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
