Responsibilities Provide family‑centered case management focused on strengthening family functioning Support children and youth in maintaining positive connections with their families Deliver culturally and linguistically responsive services to diverse populations Conduct home visits and provide individual, family, and group outreach Help families access community resources and build natural support systems Coach parents on effective communication, supervision, and nonviolent parenting strategies Advocate for families in educational, legal, and social service settings Assist in stabilizing children in foster care placements Transport children and families as needed to appointments and services Complete detailed documentation in electronic records for use by social workers, courts, and other professionals Work toward achieving service goals within established timelines Collaborate with multidisciplinary teams to coordinate care and services The Ideal Candidate You are: A strong communicator with excellent written and verbal skills Detail‑oriented and able to manage documentation effectively Compassionate and culturally responsive Comfortable working independently in the community and in family homes Knowledgeable (or eager to learn) about community resources and parenting supports Bilingual (a plus, but not required) Flexible and able to travel extensively within the service area Qualifications High School Diploma, GED, or HiSET required 0–3 years of related experience in human services, family support, or a similar field Valid Class C or Class D Driver’s License Reliable transportation with current registration, inspection, and insurance #J-18808-Ljbffr Seven Hills Foundation
