Family Nurse Practitioner or Physician Assistant

LifeLong Medical Care has an exciting opportunity looking for a Family Nurse Practitioner (FNP) or Physician Assistant (PA) for our Care Transitions Center. This is a hybrid/remote position. Candidates must live in California. We serve infants, children, and young adults from diverse ethnic and socioeconomic backgrounds. This position is represented by UAPD. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of UAPD, as defined in the CBA. Benefits Compensation: NP/PA $127,504 - $151,153.60/year depending on years licensed and pro‑rated to FTE, sign‑on bonus, and excellent benefits. Acceptance rates may vary by funding levels and applicants. Responsibilities Provide preventive and primary health services in a Family Practice Community Health Clinic setting. Act as advocate for clients. Consult with other health team members, including case managers. Maintain client records necessary for clinic reporting and billing. Perform history and physical examinations, assess clinical status, and establish and maintain medical records. Order and/or perform laboratory and screening tests; request outside consultations and evaluations as needed. Plan a program of care, formulate problem lists based on clinical findings, recommend treatment and medication, and provide follow‑up care. Provide education, verbal and written, as well as counseling to patients, families, and caregivers. Recognize and evaluate situations requiring immediate physician intervention and institute necessary treatment procedures essential for patient life. Perform other duties as assigned. #J-18808-Ljbffr
