Senior Case Manager

To support its national case management and resource-navigation efforts, the part-time Senior Case Manager will manage confidential intake, coordinate resource navigation, and assist beneficiaries in accessing public and community resources remotely, dedicating approximately 10 hours per week. Key responsibilities Conduct confidential intake and case triage, assessing needs and developing practical next steps for beneficiaries Provide high-touch support in navigating public benefits and community resources, ensuring beneficiaries understand eligibility and application processes Build and maintain relationships with service providers and community organizations to enhance referral pathways and resource accuracy Required qualifications At least 10 years of relevant experience in case management or social services Demonstrated ability to manage complex, sensitive cases independently from intake to resolution Strong knowledge of public-benefit and community-resource systems Experience conducting intake and developing service plans while maintaining accurate case documentation Proficiency in using case management systems and online resource directories
