Pay: $2,639.20 weekly


Shifts: M-F 8-5 and a minimum of one weekend a month on call

This role specializes in home health therapy services, focusing on patient evaluation and treatment based on physician referrals. Key Responsibilities of the Physical Therapist

• Perform physical therapy evaluation and treatment based on physician orders/referrals
• Complete comprehensive evaluations using appropriate tools and procedures
• Review patient records for current diagnosis and contraindications
• Develop and implement treatment plans with measurable goals
• Regularly reassess the effectiveness of treatment plans and make adjustments as needed
• Communicate patients' needs and progress to the treatment team and involved parties
• Consult with medical providers on patient progress and discharge planning
• Assist in securing adaptive equipment and integrating it into the rehab program
• Maintain and submit documentation in accordance with regulations
• Supervise Physical Therapist Assistants and Aides in patient care and activities
• Participate in staff development and continue education to enhance skills

Requirements of the Physical Therapist

• Indiana state license
• Experience in Home Health, HCHB, and OASIS
