Licensed Outpatient Physical Therapist

Job Title

Sign-On Bonus: $5,000 for eligible rehires and external hires meeting qualifications and payment conditions.

Required Qualifications:

• Bachelor's, Master's, or D.P.T. degree from an accredited Physical Therapy Program.
• Oregon Physical Therapist License upon hire.
• Physical Therapist License in state of hire and wherever care is delivered.
• National Provider BLS certification from American Heart Association upon hire.

Additional Requirements:

• Specialized physical therapy training or experience may be required occasionally to meet department needs.
