RN - Hospice & Homecare Nurse

Deliver compassionate, individualized patient care following tailored care plans designed to meet each patient's unique needs. Ensure safety principles are consistently applied and collaborate closely with physicians, case managers, and interdisciplinary teams to optimize patient outcomes.

• Provide care for hospice and homecare patients ranging from newborns to the elderly, including conducting admission assessments and developing initial care plans.
• Coordinate with patients, families, and healthcare providers to update and revise care plans as necessary.
• Supervise Home Health Aide services in accordance with organizational policies.
• Participate in on-call rotations to guarantee 24/7 nursing support for hospice, homecare, and private duty patients, including scheduled visits and admissions.
• Maintain accurate and timely documentation following agency guidelines.
• Foster positive relationships with patients, families, colleagues, physicians, and referral sources.
• Encourage patient independence by setting care goals and involving patients and families in discussions about medical care, medications, and self-care skills.
• Observe and assess patients' physical, mental, emotional, and spiritual well-being, collaborating with team members as appropriate.
• Adhere strictly to privacy and security regulations concerning Protected Health Information (PHI).

• Graduate of an accredited nursing program is required.
• Bachelor of Science in Nursing (BSN) is strongly preferred.

• Several years of clinical nursing experience.
• Previous hospice and/or home health experience is preferred.

• Current Registered Nurse licensure in the state of Kansas.
• Basic Life Support (BLS) certification required within three months of hire.
• Valid Kansas driver's license and auto insurance meeting state requirements.
• Certification as a Hospice and Palliative Care Nurse is preferred.

• Proficiency with MS Office and strong keyboarding skills.
