RN – Case Management (Contract)

• Location: California 95240

• Contract Length: 13 Weeks

• Pay: $2,448 – $2,743 per week

• Schedule: 4x10-hour shifts (40 hours/week)

• Shift: Day Shift, 8:00 AM – 6:30 PM

• Days: Variable, includes weekends

• Call: None

• Experience Required: Minimum 1 year acute care experience (Required)

• Coordinate patient care across the continuum from admission to discharge

• Collaborate with interdisciplinary teams to optimize patient outcomes

• Monitor care plans, resource utilization, and discharge planning

• Act as liaison between patients, families, providers, and payors

• Support compliance, documentation, and quality improvement efforts

• Education:

  • BSN (Preferred)

• Experience:

  • Minimum 1 year acute care experience (Required)

  • Case management experience (Preferred)

• Licensure/Certifications:

  • Active RN license in the state of practice (Required)

  • Case Management Certification (Preferred)

• Coordinate interdisciplinary care and ensure timely, appropriate treatment

• Review resource utilization, length of stay, and discharge plans with providers

• Manage complex patient populations and frequent ED utilizers

• Facilitate transfers and continuity of care across facilities

• Ensure regulatory compliance and maintain accurate documentation

• Must meet vaccination requirements (medical/religious exemptions may apply)
