About the Position

The focus is on creating a caring environment concentrated on healing and wellness. Nursing practices are based on evidence and current research as well as hospital policies and procedures. Clinical decision-making incorporates all four phases of the nursing process, critical thinking, and creativity. Nursing excellence is promoted through helping relationships among nursing staff and other disciplines. Responsibilities include assessing patient needs by performing and documenting comprehensive assessments. Consideration of the patient's culture, age, and developmental level is essential. Identifying patient risk factors and presenting complaints is part of the assessment process. Health information may need to be obtained from family or others as necessary. Data collection involves gathering objective, quantitative, and subjective, qualitative data. Initial admission assessments completed by other healthcare team members must be verified. A problem list is developed, prioritizing patient needs based on input from the inter-professional team. Designing actions and interventions relevant to the patient's culture and age is crucial. Desired outcomes are determined in coordination with the patient and family according to the medical plan of care. Patient care therapies and procedures must be fully implemented with empowerment and clinical autonomy. Procedures, treatments, and processes are to be explained thoroughly to the patient and family. Initiating actions to alleviate patient stress and discomfort is necessary. Recognizing and responding to changes in patient conditions must be communicated appropriately. Ongoing assessment and response to procedures and therapies are documented according to established standards. Tasks may be delegated appropriately while overseeing team member performance regarding patient care. Monitoring the accuracy and completeness of delegated patient care is part of the responsibilities. Identifying educational needs of patients related to prioritized problems is vital based on their level of comprehension. Facilitating the patient discharge process involves executing discharge orders timely. Potential discharges must be identified, and discharge teaching coordinated with Case Management and others. Input into patient/nurse assignments is provided considering available staffing and patient acuity. Active participation in unit-based activities supports organizational values and priorities. Promoting a positive group dynamic and healing environment through words and actions is encouraged. Assuring nursing excellence aligns with accepted standards, evidence-based practice, and a caring philosophy. Participation in department and/or unit quality improvement activities and core measures is expected. A holistic perspective regarding patient care is utilized throughout the role.
