Pay Range: $35.92 - $53.88

A successful candidate's actual pay rate will be based on several factors including relevant experience, skills, training, certifications and education.

Job Description Summary: At HSHS patient care comes first. Case Managers work to ensure our patients are provided cost-effective care in the most clinically appropriate setting by collaborating with the patient's family, physicians, interdisciplinary teams and payor representatives. The role of the Case Manager helps to ensure our patients receive high-quality care that is truly reflective of the services and care they need to heal.

Position Specifics:

• Department: Case Management
• Core Function: Nursing
• Schedule: Part-Time, 16 hours/week - Every Saturday and Sunday; 8 hour shifts
• Facility: HSHS St. Elizabeth's Hospital
• Location: O'Fallon, IL
• Compensation: Compensation that aligns with your experience

Education Qualifications

• Associate's Degree or Diploma in Nursing is required.
• Bachelor's Degree in Nursing is preferred.

Experience Qualifications

• 2 years of clinical experience is required.
• Prior case management experience is preferred.
• Quality/Process Improvement experience is preferred.

Certifications, Licenses and Registrations

• License as a Registered Nurse (RN) in the state of practice is required. Wisconsin: RNs may practice with a valid multi-state RN license as outlined in the Nurse Licensure Compact rules.
• Case Management certification (ACM, CCM, RN-BC) is preferred.
• Basic Life Support Certification (BLS) is required within 60 days of hire.
