CHAH Clinical Manager Registered Nurse - Full Time Day Shift

Home Support Coordinator

Oversees the day-to-day coordination and delivery of skilled home care for patients to ensure the highest quality of care.

Education:

• Graduate of an accredited nursing program

Experience:

• Minimum one (1) year experience in professional nursing within the last three (3) years.
• Supervisory experience preferred.
• Minimum six (6) months experience in Home Health or Hospice preferred.

Skills:

• Independent judgment, initiative and problem solving skills.
• Excellent oral and written communication skills.
• Attention to detail and efficient prioritization.
• Reliable transportation as required by nature of position.
• Basic knowledge of computer preferred.

Licensure/Certification/Registration:

• RN license active in the state of Missouri.
• BLS must be obtained within 90 days.
• Active Driver's License.
