At admission, develop the comprehensive social service evaluation for the clinical record, to include the patient’s social history, social and emotional need of the resident, working goals and treatment plan. Work as part of an interdisciplinary team of nurses, a Chaplain, doctors, social workers and home health aides. Attend, participate and facilitate interdisciplinary plan of care meetings regularly to discuss patient medical, psycho‑social, and physical needs. Assist families to find community resources and act as a liaison with other responsible parties. Have an understanding and acknowledgement of dementia and how to work with individuals with this disease at the various stages. Able to advocate for the patients’ needs. Provide supervision to volunteers and other social workers on the team. Work with other professionals in outside settings such as nursing homes, assisted livings, hospitals, etc. Visit patients on a regular basis to assess their needs and to meet plan of care goals. We Are Looking for Someone Who Is: Massachusetts Social Work License Master Degree in Social Work, LICSW preferred Clinical social work experience in health care setting a plus, preferable involving geriatrics.
