Texas Licensed Senior Care Manager RN

To support members with physical health conditions and chronic disease management, the full-time Texas Licensed Senior Care Manager RN will assess, plan, and implement complex care management activities while working remotely and collaborating with healthcare providers to ensure high-quality care outcomes. Key responsibilities Develop and continuously assess personalized care plans, collaborating with providers to address members' needs Coordinate care between members, families, and care providers to ensure appropriate person-centered services Monitor member status and care plans, revising as necessary to meet goals and address any barriers to care Required qualifications Degree from an accredited school of nursing or a Bachelor's degree in nursing 4-6 years of related experience in clinical nursing with direct patient care RN - Registered Nurse - State Licensure and/or Compact State Licensure required Preferred 7+ years of clinical nursing experience, particularly with adult populations Experience with state-government sponsored programs (Medicaid, STAR CHIP) is a plus
