Physical Therapist

This position is responsible for the evaluation, planning, administration, and supervision of Physical Therapy treatment in accordance with physician referral, state and national organizational standards, and established departmental policies and procedures. This position demonstrates the clinical competence and knowledge necessary to provide treatment appropriate to the age of the patients served. The Physical Therapist will evaluate patients' functional abilities and limitations. They will determine if intervention is needed and establish a plan of care and treatment goals. A comprehensive evaluation must be completed within the established time frame. Key limiting factors should be identified and prioritized. Treatment goals must be functional, measurable, patient-related, and reflect the key limiting factors. Collaboration with the patient and family/caregiver when setting goals is essential. Discharge planning should be initiated and modifications of referring orders recommended. The individualized treatment plan must consider the patient's physical, social, spiritual, cultural, educational, and age-specific needs. Skilled treatment must be provided in a safe manner. Ongoing effectiveness of treatment should be assessed, and the plan of care modified as indicated. Patients and family/caregivers must be involved in the treatment and decision-making process. Ongoing education should be provided to physicians, nurses, patients, family/caregivers, and the interdisciplinary team. Documentation must adhere to professional practice guidelines set by regulatory agencies. It should be completed legibly and within established time frames. Evidence-based and individualized treatment techniques must be documented. Documentation should reflect skilled intervention and comprehensive assessment of the patient's progress. Treatment goals must be reassessed according to departmental policies. Communication of pertinent patient information is vital to ensure optimal follow-through of care. Evaluation, treatment results, and patient progress must be communicated clearly and in a timely manner to all relevant parties. Responsibility should be accepted for routine or difficult problem-solving to ensure optimal patient outcome. Defending treatment rationale with standard practices or current literature is expected. Adaptability in assignments related to caseload, scheduling, and staffing shifts is required. Initiative should be taken to assist in other patient care areas, accepting responsibility for all patient care needs. The demonstration of appropriate supervision and delegation of personnel is essential for safe delivery of quality care. Supervision and delegation must comply with departmental policies and regulatory agencies. Participation in the training and orientation of new personnel is required. Competency of personnel must be ensured prior to delegation of tasks. Engagement in departmental operational activities is required. Attendance at required meetings and following up for pertinent information is necessary. Participation in departmental and facility process improvement is expected. Meeting productivity standards set by the department is essential. This position requires interaction with all levels of staff in various departments, as well as patients and families, concerning the health care of the patient. Interaction with external contacts regarding the healthcare of the patient is also required. The Physical Therapist will communicate with physician offices to report, request, or clarify information.
