Description

• Conduct thorough evaluations of patients in both inpatient and outpatient contexts and craft customized treatment plans to meet individual needs
• Administer physical therapy techniques focusing on improving mobility, strength, balance, and overall functional capacity
• Work collaboratively with an interdisciplinary team, including nurses, physicians, case managers, and other rehabilitation specialists to enhance patient outcomes
• Educate patients and their families about therapeutic goals, discharge plans, and home exercise regimens
• Meticulously document evaluations, treatments, and patient progress in alignment with institutional and regulatory guidelines
• Engage in care team rounds and interdisciplinary meetings to ensure seamless and coordinated patient care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (neurological, geriatric, etc.) highly regarded but not mandatory

Skills

• Prior experience in physical therapy is strongly preferred
• Comfortable treating patients with complex medical histories and mobility challenges
• Excellent communication and teamwork capabilities
• Proficient in utilizing digital health records and clinical documentation platforms
