SUMMARY:

As a Caregiver Outreach Specialist, you’ll expand access to Active Generations’ Caregiver Services by connecting caregivers with education, resources, support, and community-based services. This position serves as a local resource for caregivers and families while building strong relationships with healthcare providers, community organizations, and other partners.

Through individual and family support, educational programming, support groups, community presentations, and outreach activities, the Caregiver Outreach Specialist helps caregivers navigate challenges, identify available resources, and feel more supported throughout their caregiving journey.

This position would cover the following counties: Brown, Marshall, Day, Roberts, and Grant, with the potential of supporting Codington, Deuel, Hamlin, and Beadle

ESSENTIAL FUNCTIONS:

• Provide individualized support, education, resource navigation, and consultation to caregivers and families, including assistance with immediate and long-term care planning.

• Develop and facilitate caregiver support groups based on community and participant needs, including general caregiver support and condition-specific groups.

• Build and maintain collaborative relationships with healthcare providers, community organizations, service providers, and other partners serving caregivers and older adults.

• Conduct community outreach to increase awareness of Active Generations’ Caregiver Services and available resources.

• Plan and facilitate community presentations, educational programs, meetings, open houses, and other outreach activities.

• Connect caregivers with appropriate community resources, services, programs, and care options and maintain current knowledge of available resources throughout the service area.

• Facilitate evidence-based caregiver programs and obtain and maintain required program certifications.

• Provide individual, family, and group consultations and facilitate family meetings as appropriate.

• Maintain accurate and confidential documentation of participant contacts, referrals, demographics, services, program activities, and outcomes.

• Track program participation, caregiver satisfaction, outreach activities, and other information needed for program evaluation and reporting.

• Represent Active Generations at professional meetings, community events, outreach activities, and other opportunities to educate the public about caregiver services.

• Travel throughout the assigned service area and other areas as needed for participant services, outreach, meetings, training, and program activities.

• Perform additional duties as assigned by program management.

EDUCATION & EXPERIENCE:

• Bachelor’s degree in Social Work, Sociology, Human Services, Gerontology, Psychology, or a related field preferred.

• Two or more years of experience working with older adults, caregivers, individuals living with dementia-related conditions, human services, community outreach, or a related area preferred.

• Experience providing individual, family, or caregiver consultation and support preferred.

• Experience facilitating groups, educational programs, or community presentations is a plus.

• Valid driver’s license and satisfactory driving record required for community-based travel.

KNOWLEDGE, SKILLS & ABILITIES:

Strong interpersonal, communication, presentation, and relationship-building skills. Ability to establish trust and provide compassionate, participant-centered support to caregivers and families. Knowledge of or ability to learn resources and services available to caregivers, older adults, and individuals living with dementia or other chronic conditions.

Strong organizational and time-management skills with the ability to work independently, manage a geographically dispersed service area, and balance participant support, outreach, programming, documentation, and follow-up responsibilities. Comfortable facilitating individual conversations, family meetings, support groups, educational programs, and community presentations.

Ability to develop collaborative relationships with healthcare providers, community organizations, and other service partners. Comfortable using technology for remote work, documentation, communication, virtual meetings, presentations, and program tracking. Ability to maintain confidentiality, exercise professional judgment, and appropriately handle sensitive participant and family information.

PHYSICAL REQUIREMENTS & WORKING CONDITIONS:

• Regularly sit, stand, walk, bend, and reach throughout the workday.

• Use a computer, telephone, mobile device, and other office equipment for extended periods.

• Communicate effectively in person, by telephone, virtually, and in community settings.

• Frequently enter and exit a vehicle and travel between community locations.

• Transport program materials, presentation supplies, and other items weighing up to 25 pounds.

• Navigate community facilities, meeting spaces, participant locations, and other environments with varying levels of accessibility.

• Work in varying weather and environmental conditions associated with regional travel.

This is a hybrid, community-based position serving an assigned geographic area. Work may be performed remotely, at community partner locations, participant meeting locations, community events, and other approved settings. Regular regional travel is an essential component of the position, with occasional additional travel for meetings, training, or program activities. Work schedules may vary based on participant needs, support groups, community events, presentations, and program needs.
