Coordinate care and community services for Wisconsin Family Care members in the La Crosse area. Assess members’ needs, develop care plans, connect them with health and social supports, monitor their well-being, and maintain accurate case documentation. The role includes in-person member visits and travel throughout the assigned coverage area.
